This Safety Information is provided to help patients understand important considerations related to prescription treatments and compounded medications that may be made available through Reform MD Rx. Prescription Treatment Requires Provider Evaluation

Prescription treatments available through Reform MD Rx require evaluation by an appropriately licensed healthcare provider. Completing an assessment does not guarantee that a medication will be prescribed. Your provider determines whether treatment is clinically appropriate based on your health history, current medications, allergies, symptoms, risk factors, and other relevant information.

Tell Your Provider About Your Health History

Before starting treatment, provide complete and accurate information about your medical history, current and prior health conditions, allergies, prescription and nonprescription medications, vitamins, supplements, and other therapies. Tell your provider if you are pregnant, planning to become pregnant, breastfeeding, or if your health or medications change during treatment. Withholding or providing inaccurate health information can increase the risk of serious harm.

Medication Risks and Side Effects

All medications can cause side effects, and individual risks vary by medication and patient. Carefully review the medication-specific instructions, warnings, contraindications, potential drug interactions, and other safety information provided by your healthcare provider and dispensing pharmacy. Contact your provider if you develop side effects, new symptoms, or concerns about your treatment. Do not change your dose, stop treatment, combine treatments, or share prescription medication with another person unless directed by a licensed healthcare provider.

GLP-1 Treatment Safety

GLP-1 and related weight-management medications, including semaglutide- and tirzepatide-containing treatments when prescribed, may cause gastrointestinal and other side effects and may not be appropriate for every patient. Important risks can include pancreatitis, gallbladder problems, dehydration-related kidney problems, serious allergic reactions, and low blood sugar in certain patients taking other glucose-lowering medications. Some GLP-1 medications carry warnings regarding thyroid C-cell tumors and may be contraindicated for patients with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2. Your provider will determine which warnings and contraindications apply to the specific treatment prescribed to you.

Tell your provider promptly about severe or persistent abdominal pain, significant or ongoing vomiting or diarrhea, symptoms of dehydration, changes in vision, symptoms of low blood sugar, neck swelling or a neck lump, trouble swallowing, hoarseness, or other concerning symptoms. Follow the specific instructions provided with your prescription.

Peptide and Wellness Treatments

Some peptide, injectable, or wellness treatments may have limited clinical evidence for particular uses, may be prescribed for uses that are not FDA-approved, or may be available only as compounded preparations. Safety, effectiveness, contraindications, interactions, and monitoring needs vary substantially by treatment. Patients should discuss the potential benefits, limitations, uncertainties, and risks of any proposed therapy with the prescribing provider before beginning treatment.

Compounded Medications

When a compounded medication is prescribed, it is important to understand that compounded drugs are not FDA-approved. The FDA does not review compounded drugs for safety, effectiveness, or quality before they are marketed in the same manner as FDA-approved drug products. A licensed healthcare provider determines whether a compounded medication is appropriate for an individual patient, and prescriptions may be filled by an authorized pharmacy partner.

Pregnancy and Breastfeeding

Some treatments may be inappropriate during pregnancy, while trying to become pregnant, or while breastfeeding. Tell your provider before treatment if you are pregnant, may be pregnant, are planning pregnancy, or are breastfeeding. Follow your provider’s medication-specific instructions regarding contraception, pregnancy planning, discontinuation, and breastfeeding.

Medication Storage and Administration

Use, store, and dispose of medication exactly as instructed by your healthcare provider, dispensing pharmacy, and medication labeling. Do not use medication that appears damaged, contaminated, improperly stored, or different from what your pharmacy instructed you to use. Never share needles, syringes, injection devices, or prescription medication with another person.

When to Seek Emergency Care

Reform MD Rx is not an emergency medical service. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately. Seek urgent medical attention for symptoms such as difficulty breathing, swelling of the face, lips, tongue, or throat, fainting, severe or persistent abdominal pain, severe dehydration, or other symptoms that may indicate a serious reaction.
