Welcome to Reform MD Rx. These Terms of Service govern your access to and use of reformmdrx.com and the services made available through the Reform MD Rx platform. By accessing or using the website or requesting services through the platform, you agree to these Terms. If you do not agree, please do not use the website or services.

1. About Reform MD Rx

Reform MD Rx provides a technology-enabled telehealth and wellness platform through which eligible individuals may access healthcare services from licensed healthcare providers. Depending on the service requested, the platform may also facilitate coordination with pharmacies, laboratories, technology providers, payment processors, and other service providers involved in supporting your care.

Reform MD Rx does not guarantee that any particular treatment, prescription, medication, laboratory service, or other healthcare service will be appropriate or available to you. Clinical decisions are made by the licensed healthcare professional responsible for evaluating you.

2. Eligibility

You may use the services only if you meet applicable age, identity, residency, and other eligibility requirements. Telehealth services and treatment availability vary by state and depend on applicable law, provider licensure, clinical appropriateness, and pharmacy availability. You agree to provide complete, current, and accurate information regarding your identity, location, medical history, medications, allergies, health conditions, and other information requested as part of your evaluation.

3. Telehealth Services

Telehealth involves the delivery of healthcare services using electronic communications and technology when the patient and healthcare provider are not in the same physical location. Telehealth may include questionnaires, secure messaging, telephone calls, video consultations, electronic records, or other forms of remote communication.

Telehealth has benefits and limitations. In some circumstances, an in-person examination, laboratory testing, diagnostic testing, emergency evaluation, or other care may be necessary. A provider may determine that telehealth is not appropriate for you or that additional information is required before making a treatment decision.

4. No Guarantee of Prescription or Treatment

Completing an assessment, paying a fee, submitting an order, or communicating with a provider does not guarantee that you will receive a prescription or any particular treatment. All prescribing and treatment decisions are based on the independent professional judgment of the evaluating licensed healthcare provider and applicable law.

5. Prescription and Compounded Medications

If a licensed provider determines that prescription treatment is medically appropriate, a prescription may be sent to an appropriate pharmacy for fulfillment. Certain treatments offered through the platform may involve compounded medications. Compounded medications are prepared by compounding pharmacies for individual patient needs and are not reviewed or approved by the U.S. Food and Drug Administration in the same manner as FDA-approved drug products. Your provider is responsible for determining whether a prescribed treatment is clinically appropriate for you.

You must use prescription medications only as directed by your healthcare provider. Do not share prescription medications with another person. Contact your healthcare provider if you experience side effects, have questions about treatment, or believe your medical condition has changed.

6. Emergencies

Reform MD Rx is not an emergency medical service. Do not use the website, messaging features, or telehealth services for a medical emergency. If you believe you are experiencing a medical emergency, call 911 or seek immediate care from an appropriate emergency medical facility.

7. Your Responsibilities

You are responsible for providing truthful and accurate information, following your provider’s instructions, reviewing medication information provided to you, asking questions when you do not understand your treatment, and promptly informing your provider of relevant changes in your health. You are also responsible for maintaining the confidentiality of your account credentials and for activity occurring through your account.

8. Payments and Charges

You agree to pay all charges disclosed to you and authorized by you in connection with services requested through the platform. Charges may relate to clinical evaluation, treatment, medication, pharmacy fulfillment, shipping, laboratory services, or other services, depending on the program selected. Prices and available services may change from time to time, subject to applicable law.

Refunds, cancellations, and order changes are governed by the Reform MD Rx Refund & Cancellation Policy and any applicable pharmacy, provider, or service-specific requirements. Prescription and compounded medication orders may become non-cancellable or non-refundable once certain clinical or pharmacy processing steps have occurred, as described in the applicable policy presented to you.

9. Pharmacy Fulfillment and Shipping

When treatment is prescribed, medication fulfillment and shipment may be performed by a third-party pharmacy. Shipping estimates are estimates rather than guarantees and may be affected by provider approval, pharmacy processing, prescription verification, inventory, weather, carrier delays, holidays, or other circumstances outside Reform MD Rx’s reasonable control. You are responsible for providing an accurate and deliverable shipping address and for following any medication storage instructions provided by the pharmacy or healthcare provider.

10. Website Information Is Not a Substitute for Medical Care

General information appearing on the Reform MD Rx website, including information about wellness, medications, treatments, symptoms, or potential benefits, is provided for informational purposes and is not individualized medical advice. Website content should not be used to diagnose a condition, determine whether a treatment is appropriate for you, or replace advice from a qualified healthcare professional.

11. Communications

By providing contact information, you authorize Reform MD Rx and service providers acting on its behalf to communicate with you regarding your account, requested services, appointments, orders, treatment-related administrative matters, and other communications permitted by law. Marketing communications, where used, are subject to applicable consent and opt-out requirements.

12. Privacy

Your use of the website and services is also subject to the Reform MD Rx Privacy Policy and any additional privacy notices or patient-facing privacy documentation applicable to particular healthcare services. Please review those documents to understand how personal information may be collected, used, disclosed, and protected.

13. Prohibited Uses

The Reform MD Rx website and its original branding, text, graphics, design, logos, and other content are protected by applicable intellectual property laws. Except as permitted by law or with prior written authorization, you may not reproduce, distribute, modify, publish, sell, exploit, or create derivative works from protected Reform MD Rx materials.

15. Third-Party Services

The platform may rely on or link to third-party services, including healthcare providers, pharmacies, laboratories, payment processors, technology providers, shipping carriers, and other vendors. Third parties may have their own terms, policies, professional responsibilities, and privacy practices. Reform MD Rx does not control every aspect of services independently provided by third parties.

16. Availability and Changes to Services

We may modify, suspend, discontinue, or restrict features or services when reasonably necessary, including due to legal, regulatory, clinical, pharmacy, operational, or technological requirements. Treatment availability may change without notice and may differ based on your location and individual circumstances.

17. Disclaimer of Warranties

To the fullest extent permitted by applicable law, the website and non-clinical platform services are provided on an as-available basis. Reform MD Rx does not guarantee uninterrupted access, error-free operation, specific treatment outcomes, eligibility for treatment, prescription approval, or that any particular service will be available at all times. Nothing in these Terms is intended to limit rights that cannot lawfully be waived or to alter the professional duties owed to you by a licensed healthcare provider.

18. Limitation of Liability

To the fullest extent permitted by applicable law, Reform MD Rx will not be liable for indirect, incidental, special, consequential, or punitive damages arising from use of the website or non-clinical platform services. These limitations do not apply where prohibited by law and are not intended to limit liability that cannot legally be excluded, including obligations imposed on licensed healthcare professionals under applicable law.

19. Suspension or Termination

Access to the website or platform may be restricted or terminated if you violate these Terms, misuse the services, engage in fraudulent or unlawful conduct, threaten the security of the platform, or when restriction is reasonably necessary to comply with legal, regulatory, clinical, or safety requirements. Termination of platform access does not eliminate payment obligations already incurred or applicable record-retention obligations.

20. Changes to These Terms

We may update these Terms from time to time to reflect changes in our services, business practices, technology, or applicable law. The updated version will be posted on the website with a revised Last Updated date. Your continued use of the services after an update constitutes acceptance of the revised Terms to the extent permitted by law.

21. Severability
